Cures target of a single disease. The caster does not have to know what the disease is. If the target is inflicted with multiple diseases, this spell removes the most potent first.

Negates the effects of paralysis on a single target
Paralyzed: The victim is completely immobile, but may take purely mental actions. Attacks against a paralyzed creature gain a +4 To Hit. As a full round action, an attacker may perform a single melee attack against a paralyzed victim which will automatically hit and count as a critical (slide only to determine base damage). Rogues may sneak attack a paralyzed victim without the normal one round delay.
To attack the caster, the monster must succeed on a DC 12 Will saving throw. If the monster passes the check, the character can be attacked normally–otherwise, the monster will chose another target. The effect only works if there is more than one party member left. If the caster is the only character left alive, the monster will attack normally. In addition, if the caster of this scroll performs an offensive* action, all effects of this spell are negated.
Deals 6 damage as Shock if AC 15 is hit. There is no saving throw to mitigate damage.
Note: The AC 15 slide requirement does not appear on the token, but it is nonetheless required.
If cast immediately after a party member gets poisoned, the target will suffer no ill effects from that Poison. Therefore, if the target would have taken HP damage, the target takes no HP damage. If the Poison had some other instantaneous effect (e.g., death), that effect would be negated.
If not cast on the same round (or the first available turn after the Poison was administered), an instantaneous Poison effect cannot be undone,
As a miscellaneous gear token, the use of this item in the adventure is limited only by the imagination of the player and the approval of the DM.
Erratum: When ignited and tossed at a target, this item surrounds a single creature with a hazy smoke. The smoke lasts for the duration of the room. All ranged attacks targeting the victim and all ranged attacks made by the victim suffer a -4 To Hit penalty. (I.e., ranged attacks going into or out of the smoke are more difficult.) The combined process of igniting the smokestick and tossing it requires a Standard Action, but does not require an attack slide or any kind of skill check.
